Medicare in California: Your State-Specific Benefits and Free Resources

California offers unique Medicare protections including the Medigap Birthday Rule, free HICAP counseling, and Medi-Cal dual eligibility programs. Here's everything a California Medicare beneficiary needs to know.

If you're on Medicare in California, you have access to some of the strongest protections and best resources in the country. Here's what's available to you.

HICAP — Free Medicare Counseling (Your Best Resource)

The Health Insurance Counseling and Advocacy Program (HICAP) is a FREE service available to every Medicare beneficiary in California. Trained counselors can help you:

  • Understand your Medicare options and choose the right plan
  • Compare Medicare Advantage plans, Medigap policies, and Part D drug plans
  • Navigate billing problems and claims denials
  • File appeals and grievances
  • Apply for Extra Help/Low-Income Subsidy
  • Understand your rights as a Medicare beneficiary

HICAP is completely unbiased — they don't sell insurance or get commissions. They exist solely to help you. Use them before making any major Medicare decisions.

California's Medigap Birthday Rule

This is one of California's biggest advantages for Medicare beneficiaries. Every year, starting on your birthday, you have a 30-day open enrollment period to:

  • Switch to any Medigap plan of equal or lesser value
  • No medical underwriting — cannot be denied regardless of health
  • No new pre-existing condition waiting periods

Why this matters: In most states, if you miss your initial 6-month Medigap open enrollment at 65, insurers can deny you based on health or charge much higher premiums. California gives you a fresh chance every year.

How to use it: Start shopping for Medigap plans a few weeks before your birthday. Your new coverage can begin on the first of the month following your birthday.

Medi-Cal + Medicare (Dual Eligibility)

If you have limited income, you may qualify for both Medicare AND Medi-Cal (California's Medicaid program). Being "dual eligible" can dramatically reduce your healthcare costs:

Medicare Savings Programs:

  • QMB (Qualified Medicare Beneficiary): Income up to 100% FPL. Pays Part A premium, Part B premium, AND all deductibles, copays, and coinsurance
  • SLMB (Specified Low-Income Medicare Beneficiary): Income up to 120% FPL. Pays Part B premium
  • QI (Qualifying Individual): Income up to 135% FPL. Pays Part B premium

2025 Federal Poverty Level reference: $15,650/individual, $21,150/couple. So QMB covers individuals earning up to about $1,304/month.

Full Medi-Cal: If your income is low enough, you may qualify for full Medi-Cal benefits on top of Medicare, covering dental (Denti-Cal), vision, transportation, and more.

Dual Eligible Special Needs Plans (D-SNP)

These are special Medicare Advantage plans designed for people with both Medicare and Medi-Cal:

  • Coordinate all your Medicare and Medi-Cal benefits in one plan
  • Often include extensive extra benefits
  • Dedicated care coordinators who understand both programs
  • May simplify billing and paperwork
